Required Documentation:

If Graduated from an Accredited Program:

• Copies of your transcripts showing educational attainment and/or proof of highest degree earned in a healthcare-related field.
• Three letters of recommendation verifying at least 2 years of teaching experience in a healthcare-related field within the past 5 years.
  • Letter of recommendations must be on company letterhead, signed/dated, and provide the start and end dates of your teaching experience.
  • Teaching experience must be in an educational classroom setting such as a University, Career College, or Technology School.
  • Letter of recommendation must be written from a Dean, Program Director, Fellow Instructor or equivalent.

If Holding a Professional Certification:

• Proof of certification in the field of instruction from the issuing agency.
• Three letters of recommendation verifying at least 2 years of teaching experience in a healthcare-related field within the past 5 years.
  • Letter of recommendations must be on company letterhead, signed/dated, and provide the start and end dates of your teaching experience.
  • Teaching experience must be in an educational classroom setting such as a University, Career College, or Technology School.
